1. Program Description

The Foresight Health Care Management Notifications program is a notification service operated by Foresight Health, Inc. ("Foresight Health," "we," "us," or "our") in coordination with partner specialty clinics (such as neurology and endocrinology practices). If you are an existing patient of one of our partner clinics, and your clinic has shared your contact information with us for care coordination purposes, you may receive SMS messages from this program letting you know that a nurse from our care team intends to call you about a Medicare-covered care management program — such as Chronic Care Management (CCM) or Principal Care Management (PCM) — offered through your clinic. Messages may also include callback contact information and information about how to receive help. Messages do not contain protected health information about specific diagnoses, conditions, treatments, or test results.

2. Cancellation / Opt-Out

You can cancel the SMS service at any time. Just text "STOP" to the number from which you received the message. After you send the SMS message "STOP" to us, we will send you a one-time SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from this program. If you want to join again, contact your clinic or call us using the support information below.

4. Carrier Liability

Carriers are not liable for delayed or undelivered messages.

5. Message Frequency, Message and Data Rates

As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive up to 2 messages per outreach cycle, with occasional follow-up messages. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.

6. Supported Carriers

The program is available on most major U.S. wireless carriers. Service may not be available on all wireless carriers, and not all handsets are supported. Carriers are not responsible for delayed or undelivered messages.

7. Eligibility

You must be at least 18 years of age and an existing patient of a Foresight Health partner clinic to receive program messages. By receiving messages from this program, you acknowledge that your clinic provided your contact information to Foresight Health for care coordination purposes consistent with your treatment relationship and the clinic's Notice of Privacy Practices.

8. About the Sender

Foresight Health, Inc. is a management services organization that provides technology and operational support to specialty clinics. Clinical services associated with this program — including calls from nurse care managers — are delivered by Foresight Health Medical PA, an affiliated professional corporation. Both entities act on behalf of and in coordination with your treating clinic for care coordination purposes, under appropriate Business Associate Agreements where required by HIPAA.

We will not sell, share, or rent your mobile phone number, the contents of your messages, or your SMS opt-in status to any third party for marketing or promotional purposes.

10. Changes

We reserve the right to modify or terminate the program, or these Terms and Conditions, at any time, with or without notice. Continued participation in the program after changes constitutes acceptance of the revised Terms.
